Erin Wijeratne
Specialist Support Coordinator
-
Erin Wijeratne is the founder and director of Connected Support Coordination Australia. With over 15 years of experience as an Intensive Care and Emergency Retrieval Nurse, Erin brings a depth of clinical knowledge, resilience, and compassion to her work in the disability sector. Her transition into support coordination was inspired by her time working alongside individuals with complex needs, where she witnessed the life-changing impact of personalised, reliable, and values-driven support.
Driven by her belief that every person deserves to be heard, respected, and empowered, Erin established Connected Support Coordination Australia to deliver individualised, high-quality service to NDIS participants. Under her leadership, the business has grown beyond Gippsland to offer support to participants across Victoria and the Northern Territory, ensuring greater access to responsive and professional coordination services, no matter the location.
Erin’s background in high-pressure healthcare environments has shaped her strengths in crisis management, advocacy, and communication — all of which now guide her approach to NDIS navigation and participant support. She is passionate about helping people build their capacity, understand their plans, and live the lives they choose.
Natalie Weber
Support Coordinator
-
With 19 years of experience as a dedicated federal officer, I bring a strong foundation in investigation, security, and compliance—having led operations across Australia’s air and seaports to protect our borders. Throughout my career, I’ve developed sharp problem-solving skills, a keen eye for detail, and a deep commitment to clear, compassionate communication. These strengths now guide my work as an NDIS Support Coordinator, where empathy and understanding are just as vital as experience.
Born and raised in Darwin, I’m also a proud mother and devoted wife. I have personal, lived experience within the NDIS space, which gives me a unique insight into the challenges families face when navigating complex systems. This perspective fuels my passion to support others with empathy, respect, and clarity.
When I’m not working, you’ll often find me camping, fishing, gardening, or enjoying the outdoors with my family and our animals. These hobbies not only bring balance to my life but also reflect the patience, care, and connection I bring to my role every day.
I’m passionate about making a meaningful difference in the lives of those I support—helping people build confidence, access the right resources, and feel empowered in their NDIS journey.
